Here's some additional information on the crashed system.

The system has two disks an RZ26L in slot 1 (root) and a RZ28-VA in slot 0
We're using customized partitions on UFS
file system, no ADVFS.
It seems like LSM is configured on /dev/rz1g? **********correction*********

Examining the disklabel of the root disk on the backup ALpha which is
identical to the crashed one shows the following:

#disklabel -r /dev/rrz1a
# /dev/rrz1a:
type: SCSI
