Really odd error... (Take II)

Followup to what I posted about before.

Common comments seems to suggest that patch #153 from patchkit #2 is
causing the problem, it is apparently buggy. This is a 4.0D box, FYI.

There's two approaches that were suggested on how to tackle the bug.
Either #1) uninstall patch #153 or #2) install patchkit #3.

I made an attempt to uninstall the patch, however it didn't work
complaining about a bunch of file-not-founds. It seems reasonable to me
that I should put the bug-fixing Uber-patch in and be done with it.

So, silly question ... where do I get patchkit #3?

I apologize for the "newbieness" of this message. I just inherited this
box and I've not had to deal with patch-level issues in the past.

Oh, one other thing, several people wrote pointing out to me that I
violated a sacred covenant by posting the password file with the encrypted
PWD entries. Somebody even emailed my boss. Anyways, Yea, I know. I
realized about 4 seconds after I hit send that I should have sterilized
it. However, I did go to the machine in question ahd issued new passwords
to those accounts. Sorry for the heart attacks.
