good disks marked bad

I have a problem that when we put pressure on our kzpsc with an operation
like a defragmentation, one or two disks will turn on the yellow light.
When we have seen this in the past we simply replaced the disk.
Once we replaced the disk with the same disk by accident and ...
        the disk was OK.
Again we tried just downloading the configuration again to the swxcr and ...
the disk was OK and the data was OK.
We had what appeared to be a disaster two disks in the same scsi stack
in a raid configuration lit up the yellow light (during a defrag').
I just downloaded the configuration from a floppy and everything was there.
        All 120 GB were OK.
I have heard that others have had this problem (from seminars).
Is it a bad kzpsc? firmware? disk incompatibility?
DEC support says they never heard of it happenning to anyone else.
(except an old controller that could not wait for a new disk
with power save features to spin up).
