I had this same problem. There is an error in the way
root gets encapsulated. Compaq knows about this
problem. The solution to the problem is to go to
single user mode and correct the links in /etc/fdmns.
This
problem in no way effects operation of the system. The
chvol command, and others, assume a file name
structure that was not followed by the encapsulation
process.
By correcting the links in /etc/fdmns to the format
chvol complains about, you fix the problem all
together and sys_check will no longer complain. This
only happens with root encapsulation. It does not
happen when you manually create mirrored volumes.

Is this a typo? It should be
"/etc/fdmns/usr_domain/vol-rz8d", not the above.
I take it, you're using LSM? Tried checking the volume
itself?
