Linux install fdisk newbie

I want to install RH6.1 on my alpha such that I can boot from the SRM.
In order to do this, I must use fdisk instead of the DiskDruid. I need
some help figuring out the start/end cylinders for each of my
partitions. It is my understanding that I need to use the BSD labeling
in order to boot directly from SRM.
I have a 2.1GBSCSI RZ28M disk.

Here's a transcript of my fdisk session:

> begin transcript

bash# fdisk /tmp/sda
Command:p
Disk /tmp/sda: 65 heads, 62 sectors, 1019 cylinders
Units = cylinders of 4030 x 512 bytes
Command:b
Warning: starting partitions at cylinder 1 will be bad for the health of
your partition table -- start at cylinder 2 instead!
BSD disklable command:s
type:SCSI
disk:RZ28M
bytes/sector:512
sectors/track:99
tracks/cylinder:16
sectors/cylinder:1376
cylinders:2595
.
.
.

> end transcript

I want to create the following 5 partitions (in listed order):
/boot 8M
/ 100M
swap 64M
/home 400M
/usr the rest of the disk

Does anyone have any experience doing this?

Thanks in advance
-ben
