LSM setup: Internal volume daemon error

I am getting the error below when I start to set up LSM. I am
following the directions from section 3.3.2.1 of the LSM manual (yes,
I am even RTFM for once :)). OS version is 4.0F PK1.

# volinstall
LSM: Creating Logical Storage Manager device special files.
# vold -m disable
# voldctl init
# voldg init rootdg
voldg: Disk group rootdg: cannot create: Internal volume daemon error

I have tried cleaning off LSM a couple of times per the manual instructions,
without success. The LSM-OA license is installed, though I don't think it
should be needed for the initial setup. There was an identical error in
the archives but no summary, so I'm hoping someone has seen it. I looked
over the 4.0F PK4 patch list but did not see anything relevant.
