SCSI controller Memory Error Messages

Good Afternoon,

ES40 Tru64 Version 5.0A AdvFs

I have added a new controller card (Qlogic card) and the
system will not come up correctly. I receive the following messages:

   UNABLE TO ALLOCATE CONTIG MEMORY
    FOR CONSOLE FIRMWARE IMAGE


   UNABLE TO READ BACK FIRMWARE FROM CHIP

   contig_malloc CANNOT FIND MEMORY IN SAVED LIST

   Should I be identifying this card to the system somewhere?
    Any help would be appreciated.
